ICCBBA Registered Facility Identification Numbers (updated 12/14/2007)
This file contains an updated list of ISBT Facility Identification “W” numbers that will be used by United Blood Services facilities. It contains the "DBA" name for each United Blood Services center paired with its W number. In December 2007, this list was updated to reflect the FDA's new requirement that all DBA addresses be changed to the Blood Systems corporate headquarters address and single FDA registration number.
United Blood Services Map of Codabar to ISBT 128 Codes
This is a list of most of the Volunteer donor ISBT Component codes used by United Blood Services. This list does not include the ISBT codes for Autologous donations or Autologous Biohazard donations. The ISBT codes for Autologous donations will have the V replaced with a 1 and Autologous Biohazard donations have the V replaced with an X.
Example: Red Blood Cells/CPD>AS5/500mL/ResLeu<5log6 has a product code of E0336. The ISBT component codes are:
- E0336V00
- E0336X00
- E0336100
Please note that we may send you additional codes at a later date, but this is a current comprehensive list of products we produce.
Sample ISBT Validation Barcodes:
This file contains sample codes that you may use for validation purposes. These are intended for validation only. Unfortunately, a complete United Blood Services label is not included; however a sample label is included for validation purposes. It is not necessary to validate each component code, but this list provides validation barcodes for the various types of barcodes used when labeling components using ISBT criteria. An error in the sample label was corrected on 6/22/07. The eye-readable text of the ABO/Rh label was changed from “POSITIVE” to “NEGATIVE”.
